This sample form provides an example for how to approach outside activity, family member employment and other circumstances relevant to potential conflicts of interest. Companies may ask employees to certify annually that they have no conflicts of interest. If an employee cannot make that certification, the company should inquire further and determine how to best resolve the conflict. The core issue is whether an employee's personal interests conflict with his or her professional responsibilities.
